How to refer a patient or yourself for a fetal cardiology evaluation
If you would like to request a second opinion from one of our fetal cardiologists or you would like to be evaluated for a fetal cardiac intervention, please follow these steps:
Please send the completed Second Opinion Request form in an email to FCSCReferrals@childrens.harvard.edu with your patient’s name (or your name) and attach the medical records for current pregnancy: fetal echocardiogram report, OB records, demographics, and insurance information. You may also fax this to 617-730-0124.
Please upload the fetal echocardiogram images by following these instructions:
If you would like to mail the images, please send a disc to: Fetal Care and Surgery Center c/o Boston Children’s Hospital 300 Longwood Ave. Farley 2 Boston, MA 02115
When you have uploaded the fetal echocardiogram images, please reply all to this email chain so that the images can be reviewed promptly. This same link can be used to send images to Boston Children’s Department of Cardiology going forward. Please reply all to this email chain each time you send new images.
